• Clear Spring Health Managing Health Conditions

    At Clear Spring Health, we provide support to help you manage your healthcare wisely with our health and wellness incentive programs, nursing hotline, care management, and more.

    We offer care manager support through our partners for members with health conditions to help you navigate necessary appointments, screenings, scans, and prescriptions.

    Members living with ongoing conditions may be eligible for additional assistance from a nurse, case manager, or social worker.

    Care Management

    Care managers work with eligible members on a one-on-one basis to offer support by developing care plans and checking in regularly by phone, video, or home visit.

    Clear Spring Health has a contract with Medicare to offer PPO, HMO, and PDP Plans. Eon Health has a contract with the Georgia Medicaid program and a contract with the South Carolina Medicaid program. Enrollment in these plans depends on contract renewal.
