At-Home Monitoring and Real-Time Care
Through our eCare home-monitoring program, you can get the real-time and continuing medical attention you need for your chronic condition in the comfort of your home.
Continuous monitoring is a great way to see if lifestyle changes or medications are working. With home monitoring, you will have access to real-time data for clinical management and make decisions about your treatment, such as adjusting dosages or changing medications.
How Does it Work?
Clear Spring Health members who participate can receive special equipment to take their vital signs at home. The equipment will send the results to an eCare Nurse or Case Manager, who will review the readings as they come in. If the readings are outside of normal ranges, the Nurse will reach out to you for follow-up.

Living far from a clinic, not having a care, the cost of gas, not having enough health centers or transportation options can make it hard to get the care you need.
Patient–Provider Communication
Remote monitoring increases the frequency of communication between you and your care team to improve overall communication and support.

Reduced Spending
Remote monitoring can help prevent or reduce hospital visits, reducing out-of-pocket and plan benefit costs. By keeping track of your health at home, we can help you spot any problems early and prevent bigger issues that might send you to the hospital. This can help you avoid costly visits, saving both your time and money.
Equipment
Upon enrollment into the eCare Program, you can expect the following:
- An eCare Nurse and/or Care Manager will reach out to obtain verbal consent to participate.
- The necessary equipment will then be shipped to your residence.
- After delivery, a Nurse will contact you within 24 to 72 hours to go over equipment setup, and check if the device is working properly.
- Monitors, blood pressure cuffs, and batteries are replaced at no cost to the member.
Types of Equipment:
- Hypertension: iBlood pressure device for monitoring blood pressure. Smaller and larger cuffs are available. The blood pressure monitor is equipped with digital automatic SIM card and sends reading to the Nurses through a cell phone signal. You don’t need to connect to Wi-Fi or keep track of your results.
- Diabetes: iGlucometer or Continuous Glucose Monitor (Only if you meet criteria). Glucose strips are replaced at no cost. New strips are provided every 90 days.
- Chronic Obstructive Pulmonary Disease (COPD): iPulse Oximeter checks the oxygen level in your blood and sends the results to the nurses using a cell phone signal.
- Congestive Heart Failure: iScale helps you track your weight and sends the results to the Nurse using a cell phone signal.
- Monitors, blood pressure cuffs, and batteries are replaced at no cost to the member.
Equipment Return
You must return the equipment if you choose to stop participating in eCare Home Monitoring program.
Monitoring and Real-Time Interactions
Your results are monitored continuously through real-time software. Based on this, the Nurses or your Care Manager will follow clinical guidelines applicable to your specific healthcare needs.
Daily Home Monitoring is performed Monday through Friday, 8 a.m. to 7 p.m. ET, and nursing personnel are available for immediate member attention during these times. After hours, weekends, and holidays, nursing assistance is available through the nursing hotline.
Program Eligibility and Intake
You may get an invitation (call or letter) to join the remote patient monitoring program based on your answers to the Health Risk Assessment, a referral from your doctor, or if your Care Manager thinks it could help you.
If you are a health care provider and would like to refer a member to our program, please fill out the Referral Form below and send back to us through email or fax.
To participate in the eCare in-home monitoring program, you must have one or more of the following:
- Hypertension
- Diabetes*
- Chronic Obstructive Pulmonary Disease (COPD)
- Congestive Heart Failure (CHF)
*Member who are diagnosed with Diabetes are welcome to participate in the program but must be enrolled in Case Management and will be followed by their assigned Case Manager.
