The Clear Spring Health Care Management team values every member’s health and well-being. We understand that managing illness and chronic conditions can sometimes feel overwhelming and challenging.
Our free Complex Case Management program is designed to support members living with ongoing health needs. Our compassionate, experienced nurses are trained to understand each member’s particular situation. They take the time to listen, answer questions, and help members feel more confident about their care.
Members enrolled in Complex Case Management are matched with a nurse who will regularly connect with them via phone. During these calls, members can discuss their health status, medications, doctor’s instructions, and personal health goals. Together, the nurse and member create a practical plan to help manage conditions and maintain health at home.
Our goal is to provide members with the knowledge, tools, and support needed to live as well and as independently as possible.
Members enrolled in Case Management can benefit from the following:
- Disease-specific education
- Medication review and education
- Development of health goals that are important to the member
- Ongoing access to a qualified nurse for guidance and support
- Action plans developed together with your nurse to support healthy living
