Disenrolling from a Clear Health Spring Medicare Advantage Plan
Need to disenroll from a Clear Spring Health Medicare Advantage plan? Medicare has specific rules about when and how disenrollment can happen.
When can disenrollment occur?
In most cases, enrollment changes to Medicare Advantage plans can only be made
- During the Annual Enrollment Period (AEP): October 15–December 7;
- During the Open Enrollment Period (OEP): January 1–March 31; or
- During a Special Enrollment Period (SEP) if certain qualifying events occur during the year

How to disenroll

By enrolling in a different plan
If it is one of the enrollment periods listed above, signing up for and being accepted into a different Medicare Advantage or Part D plan will generally result in automatic disenrollment from a Clear Spring Health Medicare plan. No additional action is usually required.

By returning to Original Medicare
If it is one of the enrollment periods listed above and the decision is to return to Original Medicare, complete the disenrollment form below, sign it, and send it

If it is one of the enrollment periods listed above and the decision is to return to Original Medicare, per the Centers for Medicare & Medicaid Services (CMS) requirements, complete the disenrollment form below, sign it, and send it.
After a disenrollment request is submitted, all medical care must continue to be received through Clear Spring Health until the effective date of disenrollment. Please contact us to confirm disenrollment before seeking services outside of the Clear Spring Health network. Notification of the effective date will be sent after the completed form is received and processed.
Losing coverage (automatic disenrollment)
In certain situations, disenrollment from a Clear Spring Health Medicare Advantage plan may occur automatically, such as when a member
- moves their permanent residence outside of the plan’s service area (including incarceration);
- loses entitlement to Medicare Part A and/or is no longer enrolled in Medicare Part B; or
- is accepted into a new Medicare Advantage plan; or
- passes away.
Whenever automatic disenrollment occurs, we will provide proper notice and an opportunity to appeal the decision before disenrollment takes effect.
